TO All Internal Revenue Officials, Employees and Others Concerned
Notice is hereby given of the loss of six (6) sets of unused/unissued BIR Form No. 2298 -- Product Replenishment Certificate, bearing the following serial numbers:
PRC201600002094 - PRC201600002099
Asuncion, Revenue Officer I, Export Permit and Product Replenishment Section The abovementioned forms were reported as lost by Ms. Angelie Monica H.
cancelled. All official transactions involving the use of said forms are therefore considered as INV ALID. Excise Large Taxpayers Field Operations Division, and have consequently been
the necessary measures to prevent the improper or fraudulent disposition or use of the promptly notify this Office in the event that the aforesaid forms are found and to take Internal Revenue officials, employees and others concerned are requested to
