TO All Interaa! Revenue Officials, Employees and Others Concerned
together with other pertinent information, to wit: Notice is hereby given of the loss of the accountable forms hereunder listed

The abovementioned forms were lost during Marawi siege in Marawi City, as reported by Ms. Norhata M. Ibrahim, Revenue Officer I-Collection Agent, Revenue are therefore considered as INVALID. consequentiy been cancelled. All official transactions involving the use of said forms District Office No. 102, Revenue Region No. 16, Cagayan de Oro City and have
Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
