AMENDMENT TO PHILHEALTH CIRCULAR NO. 04, S. 2008, SUBJ: GUIDELINES ON THE IMPLEMENTATION OF THE MORATORIUM ON THE PROCESSING OF RENEWAL OF ACCREDITATION OF INSTITUTIONAL HEALTH CARE PROVIDERS (IHCP'S)
PhilHealth Circ No. 4 s. 2008, item no. 8 states that all hospitals and ASCs in regions VII, VIII, IX, X, XI, XII CARAGA, ARMM & CAR shall be given an extension for filing of applications for the renewal of accreditation until April 15, 2008.
The said extension is also applicable to out patient clinics, namely, Free Standing Dialysis Clinics, Out Patient Benefit Package Providers, Maternity Care Providers and Anti-TB/DOTS Package Providers in alf regions except for regions VII and VIII.
