AMENDMENT TO CIRCULAR 46, S. 1999

The following amendment to PhilHealth Circular No. 46, s-99 shall take effect immediately. 1. For partial disability pensioners of the Social Security System (SSS)

The photocopy of the Bank Account Passbook is no longer honored as a supporting document when availing Medicare benefits. Instead, a copy of the certification from SSS starting the period covered by the pension should be attached to PhilHealth Form 1.

2. For GSIS Members (Permanent/Temporary/Casual)

Original and properly accomplished PhilHealth Form 1 is sufficient. In case it is not properly accomplished, the claim will be returned to the hospital for completion. The first page of the copy of the Policy Contract, Updated Service Record and Certificate of Employment need not be submitted.

All other provisions not inconsistent herewith remain in full force and effect.
