REVISED FEES FOR VACCINATION SERVICES

In compliance with Memorandum Circular No. 121 dated November 2, 1990 of the President of the Philippines directing all departments, bureaus, and agencies of the government to revise fees and charges at just and reasonable rates sufficient to cover administrative costs, the revised fees on vaccinations are as follows:

Yellow Fever vaccination

Cholera vaccination

c.

Other vaccination

- 20.00

d.

Issuance of International Certificate of Vaccination to

replace lost certificate

- 20.00

e.

Issuance of Certificate

of Waiver of Vaccination

(due to pregnancies, allergies, etc.)
