COLLECTION OF COMPUTER FEES FOR MAIDRS

Upon the successful completion of the functional and operational testing, Certificates of Acceptance of an IT Facility (CAIF's) were issued for the Manufacturers, Assemblers, Importers and Dealers Reporting System (MAIDRS) modules in the attached schedule at the sites and on the dates stated.

Accordingly, pursuant to the LTO Information Technology Project Build-Own-Operate Agreement and the UAT Plan, you are hereby directed to commence collection of computer fees for the issuance of CSR for the following modules of MAIDRS applicable to your respective sites as follows:

All orders/memoranda in conflict herewith are deemed superseded.
