REQUIREMENTS IN THE APPLICATION OF DUPLICATE AND REPLACEMENT PLATES, DUPLICATE OFFICIAL RECEIPT (OR)/CERTIFICATE OF REGISTRATION (CR) AND REPLACEMENT STICKER

In view of the recent carnapping cases and for purposes of uniformity, security and protection of both the vehicle owner and issuing officials, you are hereby directed to require the following documents listed hereunder in the Application of Duplicate and Replacement Plates, Duplicate Official Receipt (OR)/Certificate of Registration (CR) and Replacement Sticker, to wit:

A. Duplicate Plates

Certified xerox copy of Registration Certificate

Certified xerox copy of Official Receipt

Affidavit of Loss executed by registered owner

Clearance from HPG/PNP

Authority of the registered owner

Two (2) valid ID of registered owner

Two (2) valid ID of authorized representative

Motor Vehicle Inspection Report (MVIR)

Certification from the issuing DO/EO that no application for duplicate was effected

B. Replacement of Plates

Certified xerox copy of Registration Certificate

Certified xerox copy of Official Receipt

LTFRB Decisions granting the provisional authority of CPC, if, For-hire

Franchise Confirmation/Verification issued by LTFRB in the Region, if, For-hire

Pair of Plates to be surrendered

Motor Vehicle Inspection Report (MVIR)

Certification from the issuing DO/EO that no application for replacement was effected

C. Duplicate OR/CR

Certified Xerox copy of Official Receipt (OR)/Certificate of Registration (CR)

Affidavit of Loss executed by registered owner

Clearance from HPG/PNP

Authority of the registered owner

Two (2) valid ID of registered owner

Two (2) valid ID of authorized representative

Motor Vehicle Inspection Report (MVIR)

Request for Confirmation from the issuing DO/EO

D. Replacement Stickers

Certified Xerox copy of Official Receipt/Certificate of Registration (CR)

Affidavit of Loss executed by registered owner

Clearance for HPG/PNP

Authority of the registered owner

Two (2) valid ID of registered owner

Two (2) valid ID of authorized representative

Motor Vehicle Inspection Report (MVIR)

Affidavit of undertaking that LTO personnel shall not be held liable for any third party claim

Previous instructions/orders/memoranda in conflict herewith are deemed amended/superseded accordingly.

This Memorandum Circular shall take effect immediately.
