ADDITIONAL TESTING FEE REQUESTED BY FATL FOR INCLUSION IN THE DOE REVISED SCHEDULE

We respectfully forwarded the attached request of FATL for an inclusion of a new fee in the DOE Revised Schedule of Fees and Charges per Joint DBM-DOF Circular No. 2-94.

Test Fees for International Clients

— P30,250.00/unitexcluding freight and customs charges

In response to the foreign query of FATL, payment of this testing fee will be coursed through the DOE Treasury Division who will issue an official receipt and prepare the corresponding report of collections. Furthermore, this fee will be treated as Income in the Special Account for General Fund 151.
