[ DOH MEMORANDUM CIRCULAR, August 21, 1963 ]

TO ALL AIRLINES, SHIPPING COMPANIES, TRAVEL AGENCIES, AND OTHERS CONCERNED

Effective immediately please inform all passengers bound for abroad coming to the clinic of this Office for immunization to present identification cards prior to their immunization. The reason for this is the fact that certain persons have presented themselves to this Office for immunization in lieu of the real passenger. In this regard, passports, re-entry permits, aliens' certificates of registration and residence certificate are acceptable.

Cooperation of all concerned shall be highly appreciated.
