[ LTO MEMORANDUM CIRCULAR NO. 522-2004, June 25, 2004 ]
PRESENTATION OF VALID IDENTIFICATION CARDS
For purposes of security and uniformity, you are hereby directed to require at least two (2) valid identification cards hereunder listed before issuing a duplicate Official Receipt (OR) and/or Certificate of Registration (CR):
1. Valid Driver's License; 2. Voter's ID Card; 3. Postal ID Card; 4. PRC ID; 5. Passport; 6. Credit-card type Government ID bearing both the picture and the signature of the holder/owner; 7. Senior Citizen ID; 8. School IDs accompanied by current registration card; 9. Company ID bearing the both the picture and the signature of the holder; and 10. SSS ID.
All orders and/or memoranda that are in conflict herewith are hereby superseded.
This Memorandum Circular takes effect immediately.
