POLICY GUIDELINES ON THE CHANGE OF BIRTH DATE OF MEMBERS

RESOLVED, to APPROVE the following Policy Guidelines on the Change of Birth Date of members:

Changes in the date of birth shall only be given due course upon presentation by the requesting member to GSIS of a Court Order attesting to the validity of such change; and

Changes in the computation of life insurance benefits resulting from the amendment in the date of birth shall no longer be entertained unless the request for such change in the date of birth is effected within one year from the issuance of his life insurance policy.

RESOLVED FURTHER, to DIRECT the Vice President (Public Affairs) to publish in the Official Gazette and in newspaper of general circulation the foregoing policy guidelines adopted on the Change of Birth Date.
