MODIFIED SCHEDULE OF FEES AND WAIVER ON THE ADDITIONAL US$15,000.00 REQUISITE DEPOSIT

Pursuant to the PLRA Board Resolution No. 14 dated 27 March 2003, series of 2003, the Authority’s Schedule of Fees is modified as follows:

I. Former Filipinos

Requisite Deposit : US$1,500.00 to include the spouse and children regardless of the number

Processing Fee : US$300.00 for each dependent

Service Fee : US$ 300.00 for each dependent

II. Other Nationalities

Requisite Deposit : US$50,000.00 for 50 years old and above US$75,000.00 for 35-49 years old

Processing Fee : US$1,500.00 for principal applicant regardless of age

Service Fee : US$300.00 for each dependent

III. Issuance of Identification Card for the 1st year is free. But upon renewal, the amount of US$10.00 each will be charged.

Likewise, the additional requisite deposit of US$15,000.00 for each dependent in excess of two (2) is waived for former Filipinos.

The Department Manager III of the Resident Retiree Servicing Center, shall be responsible for the dissemination of this Circular.

This Circular takes effect immediately.
