REPORTING OF PRIVATE HOSPITAL/CLINIC ADMISSION/TREATMENT OF PATIENTS DUE TO DRUG ABUSED AND DRUG-RELATED AILMENTS

Pursuant to the powers vested in the Dangerous Drugs Board under Section 36(a) and (1), Article VIII of R.A. 6425, as amended, the following requirements are hereby prescribed:

SECTION 1. All private hospitals/clinic and physicians accredited by the Dangerous Drugs Board shall within the first ten (10) days of the month, submit to the Board a written report of all cases of drug abuse and drug-related ailments admitted/treated during the next preceding month.

SECTION 2. In order to insure uniformity and clarity of reporting, the DDB Central Case Registry (CCR) Form No. 2 shall be followed.

SECTION 3. Failure to comply with this regulation shall subject the person/entity concerned to administrative sanctions.

SECTION 4. This Regulation shall take effect after fifteen (15) days following the completion of its publication in the Official Gazette.
