NOTICE OF LOSS AND CANCELLATION OF TCC NOS. 00121003 & 001222236

Please be informed that the unused Bureau of Customs Tax Credit Certificates (Negotiable) with serial Nos. 00121003 and 001222236 are missing as per Affidavit of Ms. Evelyn C. Difuntorum dated July 17, 1993 (copy attached).

The above Tax Credit Certificates are hereby considered null and void and the Bureau of Customs is directed not to honor and accept the same in payment of any liability to the government.

In view of the above, it is requested that the said Tax Credit Certificate be seized for investigation and all Collectors of Customs are hereby advised to issue a certification whether the aforesaid Tax Credit Certificates were not yet utilized.

Your prompt action on this matter will be highly appreciated.
