[ DOH MEMORANDUM CIRCULAR, June 15, 1965 ]
TO ALL AIRLINES, SHIPPING COMPANIES, AND TRAVEL AGENCIES
Effective immediately, please be informed that prospective passengers requesting for immunization should present to the quarantine medical of officer on clinic duty their respective I.D. cards. The following documents are acceptable for identification purposes:
a) Passports
b) Alien certificate of registration
c) Driver's license or residence certificate
d) ID card issued by schools, colleges, government offices and private companies.
