REPUBLIC OF THE PHILIPPINES DEPARTMENT OF FINANCE BUREAU OF INTERNAL REVENUE 24 April, 2003 REVENUE MEMORANDUM CIRCULAR No. 24-2003 SUBJECT : Missing One (1) original copy of unused BIR Form No. 0421 � Tax Verification Notice TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the missing one (1) original copy of an unused Tax Verification Notice (BIR Form No. 0421) with the hereunder stated serial number, to wit: TVN200200204486 The abovementioned form was reported as missing by MS. SONIA M. ATIP, of BIR-Administrative Division, Revenue Region No. 14, Candahug, Palo, Leyte, and has consequently been cancelled. All official transactions involving the use of said form is therefore considered as INVALID. Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid form is found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
