REPUBLIKA NG PILIPINAS KAGAWARAN NG PANANALAPI KAWANIHAN NG RENTAS INTERNAS Quezon City May 11, 2005 REVENUE MEMORANDUM CIRCULAR No. 20 - 2005 SUBJECT : Missing six (6) sets of unissued BIR Form No. 0423- Apprehension Slip TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the missing six (6) sets of unissued BIR Form No. 0423- Apprehension Slip (APS) with the following serial numbers: APS200200008503 � APS200200008508 The abovementioned forms were reported as missing by Ms. Marilou G. Sapungan , Revenue District Office No. 48, West Makati, Revenue Region No. 08, Makati City, and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
