Revisions in the bio-data of directors/officers of banks/NBQBs required to be submitted under Circular Letter dated 9 January 2001

CIRCULAR LETTER Series of 2001

To     :     All Banks/NBQBs

The Monetary Board in its Resolution No. 427 dated 15 March 2001, approved the revision in the bio-data form of directors/officers of banks/NBQBs required to be submitted under Circular Letter dated 09 January 2001, as follows:

a.   Deletion of disclosure of relatives within the 3rd and 4th degree of consanguinity or affinity, legitimate or common-law.

b.   The required schedule of business establishment is limited to those where relatives within the 2nd degree have equity interest, etc.

We have transmitted via cc: Mail the revised program in excel format together with the user’s manual.  Banks without cc:  Mail may obtain a diskette copy of the Bio-data Excel template from the Supervisory Reports and Studies Office.  Banks opting to submit the bio-data in hard copy, may use the attached revised format.
