Further to Circular Letter dated October 13, 1997 on captioned subject, the report forms Consolidated Statement of Income and Expenses (Annex B) and Control Prooflist (Annex A) should be numbered TB Form 5 and TB Form 5A instead of TB Form 3 and TB Form 3A, respectively.
