TO : All Regional Directors, Revenue District Officers, Employees and Others Concerned ______________________________________________________________________________ Notice is hereby given for the loss of one (1) original copy of Tax Credit Certificate (TCC) to wit: TCC No./Serial No. Date of Issue TCC Face Amount TCC Balance P194,350.24 R-018309 October 27, 2008 P527,105.25 The above- mentioned TCC was reported as lost by MR. TEODORO G.GALICIA, Revenue District Officer, RDO No. 49, North Makati on July 27, 2009. Consequently, the entire set bearing the said TCC No. is hereby cancelled. All official transactions involving the use of the said form effective July 28, 2009 is therefore considered as INVALID. Internal revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid TCC is found and to undertake necessary measures or actions in order to prevent the improper or fraudulent disposition/use of the same.
