REPUBLIC OF THE PHILIPPINES DEPARTMENT OF FINANCE BUREAU OF INTERNAL REVENUE Quezon City October 12, 2009 REVENUE MEMORANDUM CIRCULAR NO.61-2009 SUBJECT : Loss of the duplicate to quintuplicate copies from one (1) unused set of BIR Form No. 2524 � Revenue Official Receipt TO : All Internal Revenue Offic ials, Employees and Others Concerned Notice is hereby given of the loss of the duplicate to quintuplicate copies from one (1) unused set of BIR Form No. 2524 � Revenue Officia l Receipt (ROR), with the hereunder stated serial number, to wit: ROR200802403309 The abovementioned form was reported as lost by Ms. Maria Jobelle O. Morallos, Revenue Officer I, Revenue District Office No. 87, Revenue Region No. 14, Government Center, Candahug, Palo, Leyte. Consequently, the entire ROR set bearing said serial number is hereby cancelled. All official transactions involving the use of said form is therefore considered as INVALID. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid accountable form is found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
