OBJECTIVE This Order reformats and prescribes the use of the following numbered Access Request forms in compliance with Revenue Administrative Order No. 2 � 2002, dated June 14, 2002 delegating to the Chief, Security Management Division the authority to approve the same : � BIR Form No. 0033 Special Access Request Form Annex A � BIR Form No. 0034 Extension of Special Access Request Form Annex B � BIR Form No. 0035 Regular Access Request Form Annex C � BIR Form No. 0037 Revocation of Access Request Form Annex D II. REPEALING CLAUSE The reformatted forms prescribed in and made an integral part of this Order replace the old forms originally prescribed in Revenue Memorandum Order No. 2-99 dated November 27, 1998.
