REPUBLIC OF THE PHILIPPINES DEPARTMENT OF FINANCE BUREAU OF INTERNAL REVENUE Quezon City March 03, 2011 REVENUE MEMORANDUM CIRCULAR No. 14-2011 SUBJECT : Loss of several BIR Form No. AF51 � Accountable Form 51 TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of the accountable forms hereunder listed together with other pertinent information, to wit: ACCOUNTABLE FORM SERIAL NUMBER/S QUANTITY BIR Form No. AF51 ACF201000090751- 42 duplicate and ACF201000090792 triplicate copies (Used) (Accountable Form 51) ACF201000090793- 8 sets (Unused) BIR Form No. AF51 ACF201000090800 (Accountable Form 51 The loss of the said accountable forms was reported by Ms. Solita C. Mauricio, Revenue Officer III, Revenue District Office No. 41, Revenue Region No. 7, Quezon City. Consequently, the duplicate and triplicate copies of the abovementioned forty-two (42) sets of AF51 should be verified if found; whereas the eight (8) unused sets have been cancelled and all official transactions involving the use thereof are considered as INVLAID. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
