REPUBLIC OF THE PHILIPPINES DEPARTMENT OF FINANCE BUREAU OF INTERNAL REVENUE Quezon City November 21, 2008 REVENUE MEMORANDUM CIRCULAR No. 79-2008 SUBJECT : Missing duplicate copy from one (1) set of an unused BIR Form No. 0421- Tax Verification Notice TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the missing duplicate copy from one (1) set of an unused BIR Form No. 0421 � Tax Verification Notice (TVN) with the hereunder stated serial number, to wit: TVN200200013044 The abovementioned copy was reported missing by Mr. Frumencio A. Belisario, Assistant Revenue District Officer, Revenue District Office No. 07, Revenue Region No. 02, Cordillera Administrative Region, Baguio City. Consequently, the entire TVN set bearing the said serial number is hereby cancelled. All official transactions involving the use of said form is therefore considered as INVALID. Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid form is found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
