REPUBLIKA NG PILIPINAS KAGAWARAN NG PANANALAPI KAWANIHAN NG RENTAS INTERNAS Quezon City June 21, 2006 REVENUE MEMORANDUM CIRCULAR No. 36-2006 SUBJECT : Lost of forty-four (44) unissued sets and the duplicate to quadruplicate copies from another six (6) issued sets of BIR Form No. 2524 � Revenue Official Receipt TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of forty-four (44) sets and the duplicate to quadruplicate copies from another six (6) issued sets of BIR Form No. 2524 Revenue Official Receipt (ROR) with the following serial numbers: ISSUED UNISSUED ROR200502122951-ROR200502122956 ROR200502122957-ROR200502123000 The abovementioned forms were reported as lost by Ms. Maria Reina A. Edulan, Revenue Collection Officer, Revenue District Office No. 84, Revenue Region No. 13, Cebu City, Consequently, all official transactions involving the use of the missing copies corresponding to the issued RORs should be verified. With respect to the unissued lost RORs, the same have been cancelled and all official transactions involving the use thereof are considered as INVALID. Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
