REPUBLIC OF THE PHILIPPINES DEPARTMENT OF FINANCE BUREAU OF INTERNAL REVENUE Quezon C ity May 27, 2008 REVEN UE MEMORANDUM CIRCULAR No. 63-2008 SUBJECT : Missing O ne (1) Set of an Unused BIR Form No. 1954 � Certificate Authorizing Registration TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the missing one (1) set of an unused BIR Form No. 1954 - Certificate Authorizing Registration (CAR) with the hereunder stated serial number, to wit: CAR200700095500 The abovementioned form was reported as missing based on the Affidavit executed by Ms. Maria Josefa T. Lee, OIC-Administrative O fficer, Revenue District Office No. 82, Revenue Region No. 13, Cebu City. Consequently, the entire CAR set bearing the said serial number is hereby cancelled. All official transactions involving the use of said form is therefore considered as INVALID. Internal Revenue O fficials, employees and others concerned are requested to promptly notify this O ffice in the event that the aforesaid form is found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the sa me.
