previously designated as Revenue District Officer, Revenue District Office No. 71. Revenue Region No. l1, Iloilo City and have consequent!y been cancelled. All official transactions involving the use of said form is therefore considered as INVALID. The abovementioned form was reported as lost by Mr. Alexander P. Laroza,

promptly notify this Office in the event that the aforesaid form is found and to take the Internal Revenue officiais, empioyees and others concerned are requested to
