Notice is hereby given of the loss of one (1) unused set of BIR Form No. 0423 Apprehension Slip (APS) with the hereunder stated serial number, to, wit:

The abovementioned form was reported as lost by Ms. Josephine D. Madera. Chief Revenue Officer II, National Investigation Division and have consequently been cancelled All official transactions involving the use of said form is therefore considered as INV ALID.
