and have consequently been cancelled. All official transactions invoiving the use of said forms Revenue Officer II (A), Revenue District Office No. 41, Revenue Region No. 7B, East NCR are therefore considered as INVALID. The abovementioned forms were reported as lost by Mr. Asgar C. Macabanding.
