AL., ,. x- ____ _ _____ - ~~s~~~~ent~�- _ M_AY_2_~ ~~~~~ ____ x RESOLUTION On March 29, 2023, the Court issued a Resolution granting petitioner a last opportunity to submit to the Court a compliant Judicial Affidavit of Mr. Johnny Sy Co and a Verification and Certification on Non-Forum Shopping compliant with the 2004 Rules on Notarial Practice, within five (5) days from notice. On April 11 , 2023, petitioner filed a Motion for Extension of Time to File Additional Compliance and requested for an additional period of twenty (20) days or until April 29, 2023 within which to submit the requested documents. The Court granted the aforesaid motion and gave petitioner until May 2, 2023 within which to comply with the Resolution dated March 29, 2023. On April26 , 2023, petitioner filed via an accredited courier an Additional Compliance attaching thereto an Amended Judicial Affidavit and a Verification and Certification of Non- Forum Shopping. Upon a final review of the documents attached to the Additional Compliance of petitioner, the Court finds that the Verification and Certification on Non-Forum Shopping is still not compliant with the 2004 Rules on Notarial Practice and is a mere photocopy.
� RESOLUTION CTA Case No. 11024 WHEREFORE, for failure to comply with the Minute Resolution of the Court dated December 14, 2022 and the Resolution dated March 29, 2023, giving the petitioner a last opportunity to submit a compliant Verification and Certification Against Non-Forum Shopping, the Petition for Review filed by Petitioner on November 8, 2022, is hereby DISMISSED.
