OBJECTIVES This Order is issued to prescribe the use of the following numbered and reformatted Access Requests Forms: � BIR Form No. 0043 Request for System Access Form Annex A � BIR Form No. 0044 Electronic Filing and Payment System Annex B Access Request (eFPS) Form II. REPEALING CLAUSE The reformatted BIR Form No. 0043 prescribed in this order renders obsolete BIR Form No. 0033-Special Access Request Form and BIR Form No. 0035-Regular Access Request Form. The reformatted BIR Form No. 0043, dated July 2004, supersedes BIR Form No. 0043 dated February 2002. III. EFFECTIVITY This Order takes effect immediately. (Original Signed) GUILLERMO L. PARAYNO, JR.
