REPUBLIC OF THE PHILIPPINES DEPARTMENT OF FINANCE BUREAU OF INTERNAL REVENUE Quezon City 14 October, 2003 REVENUE MEMORANDUM CIRCULAR No. 67-2003 SUBJECT : Lost Two (2) pads and Fourty nine (49) sets of unused and cancelled, BIR Form No. 2524 - Revenue Official Receipts TO : All Internal Revenue Officials, Employees and others Concerned Notice is hereby given of the loss of two (2) pads and fourty nine (49) sets of Revenue Official Receipts (BIR Form No. 2524) with the following serial numbers: UNUSED ROR200300198015 - ROR200300198050 - 36 sets ROR 200300198051 - ROR200300198100 - 1 pad ROR200100523490 - ROR200100523500 - 11 sets ROR200100523501 - ROR200100523550 - 1 pad CANCELLED ROR200300198012 - ROR200300198013 - 2 sets The abovementioned forms were reported as lost by MR. RAIMUND L. LEGION, of Revenue District Office No. 85, Revenue Region No. 14, Candahug, Palo, Leyte, and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
