REPUBLIC OF THE PHILIPPINES DEPARTMENT OF FINANCE BUREAU OF INTERNAL REVENUE Quezon City July 16, 2009 REVENUE MEMORANDUM CIRCULAR NO.39-2009 SUBJECT : Loss of four (4) original copies of Tax Credit Certificates issued to HJR INTERNATIONAL CORPORATION TO : All Regional Directors, Revenue District Offic ers, Employees and others Concerned Notice is hereby given of the loss of four (4) original copies of Tax Credit Certificates (TCCs) to wit: SERIAL NUMBER DATE OF ISSUE AMOUNT R-017400 January 29, 2008 Php 103,699.09 R-017401 January 29, 2008 Php 90,300.53 R-017402 January 29, 2008 Php 106,517.89 R-017403 January 29, 2008 Php203,291.79 The above-mentioned TCCs were reported as lost by MS. CARMEN M. CAMASURA of HJR GROUP OF COMPANIES. Consequently, the entire TCC set bearing the said serial numbers are hereby cancelled. All official transactions involving the use of said form is therefore considered as INVALID. Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid TCCs are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
