REPUBLIC OF THE PHILIPPINES DEPARTMENT OF FINANCE BUREAU OF INTERNAL REVENUE Quezon City December 10, 2003 REVENUE MEMORANDUM CIRCULAR NO. 3-2004 SUBJECT: Loss of One (1) Tax Credit Certificate TO : All Internal Revenue Officials, Employees and others Concerned. ________________________________________________________________________ Notice is hereby given of the loss of the Tax Credit Certificate with the following data: TAXPAYERS NAME SERIAL NUMBER ASIA TRADERS INSURANCE CORPORATION 000959 The abovementioned TCC was reported as lost by Ms. Salvacion Ana R. Cruz, Accounting Manager of above named taxpayer, and have consequently been cancelled. All official transactions involving the use of said TCC are therefore considered as INVALID. Internal revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid TCC is found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
