Notice is hereby given of the loss of eight (8) sets of unused BIR Form No 0535 -- Taxpayer Information Sheet, bearing the following serial numbers:

TIS201400239393 -- TIS201400239400

Revenue Officer I-Assessment, Revenue District Office No. 26, Revenue Region No. 5. The abovementioned forms were reported as lost by Mr. Gerardo B. Dizon.

involving the use of said forms are therefore considered as INVALID. Caloocan City and have consequentiy been cancelled. All official transactions
