REPUBLIC OF THE PHILIPPINES DEPARTMENT OF FINANCE BUREAU OF INTERNAL REVENUE Quezon City December 19, 2006 REVENUE MEMORANDUM CIRCULAR No. 8-2007 SUBJECT : Missing 1 set, and the duplicate and triplicate copies from another (1) set of unused BIR Form No. 0423- Apprehension Slip TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the missing one 1 set, and the duplicate and triplicate copies from another (1) set of unused BIR Form No. 0423 � Apprehension Slip (APS) with the following serial numbers: APS200200022255 � one (1) set APS200200005009 � duplicate and triplicate copy The abovementioned forms were reported missing by Ms. Evelyn C. Ignacio, Accountable Forms Officer, Revenue Region No. 05, Valenzuela City. Consequently, the entire APS sets bearing the said serial numbers are hereby cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
