Notice is hereby given of the loss of one (1) set of used but unissued BIR Form No. 23 10 -- Tax Credit Certificate with the hereunder stated serial number:

TCC200600004229

The abovementioned form was reported as lost by Atty. Clarissa J. Virtudes Babaran, Revenue Attorney HII assigned at Litigation Division and Atty. Felix Paul R. Velasco HlI, Head Revenue Executive Assistant, Enforcement and Advocacy Service and has consequently been cancelled. All official transactions involving the use of said form is therefore considered as INV ALID.

Internal Revenue officials. employees and others concerned are reguested to promptly notify this Office in the event that the aforesaid form is found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the Same.
