REPUBLIC OF THE PHILIPPINES DEPARTMENT OF FINANCE BUREAU OF INTERNAL REVENUE Quezon City October 21, 2009 REVENUE MEMORANDUM CIRCULAR NO.62-2009 SUBJECT : Lost Five (5) sets of an unused BIR Form No. 1954 - Certificate Authorizing Registration TO : All Internal Revenue Offic ials, Employees and Others Concerned Notice is hereby given of the loss of five (5) sets of an unused BIR Form No. 1954 � Certificate Authorizing Registration, bearing the following serial numbers: CAR200800155396-CAR200800155400 The abovementioned forms were reported as lost by Ms. Cynthia S. Calleja, Revenue Officer I, Revenue District Office No. 68, Revenue Region No. 10, Legazpi City, and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary meas ures to prevent the improper or fraudulent disposition or use of the same.
