REPUBLIC OF THE PHILIPPINES DEPARTMENT OF FINANCE BUREAU OF INTERNAL REVENUE July 31, 2009 REVENUE MEMORANDUM CIRCULAR N o. 46-2009 SUBJECT : Missing three (3) sets of an unused BIR Form No. 1954 - Certificate Authorizing Registration TO : All Internal Revenue Offic ials, Employees and Others Concerned Notice is hereby given of the missing three (3) sets of an unused BIR Form No. 1954 � Certificate Authorizing Registration (CAR), bearing the following serial numbers: CAR200900018521 CAR200900018522 CAR200900018523 The abovementioned forms were reported as missing by Ms. Lodiminda P. Rom, Chief, Administrative Section, Revenue District Office No. 54A, Revenue Region No. 09, San Pablo City, and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
