REPUBLIKA NG PILIPINAS KAGAWARAN NG PANANALAPI KAWANIHAN NG RENTAS INTERNAS Quezon City August 31, 2005 REVENUE MEMORANDUM CIRCULAR No. 47-2005 SUBJECT : Missing duplicate copy from one (1) set of an unissued BIR Form No. 1954- Certificate Authorizing Registration TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the missing duplicate copy from one (1) set of an unissued Certificate Authorizing Registration (BIR Form No. 1954) with the hereunder stated serial number, to wit: CAR200400331819 The abovementioned copy was reported missing by Ms. Ma. Wevinia S. Perez, Taxpayer Service Section, Revenue District Office No. 33, Revenue Region No. 06, Manila. Consequently, the entire CAR set bearing the said serial number is hereby cancelled. All official transactions involving the use of said form is therefore considered as INVALID. Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid form is found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
