Employees and Others Concerned Notice is hereby given of the loss of the accountable forms hereunder listed together with other pertinent information, to wit: Accountable Form Serial Number Ouantitv BIR Form No. 2524 ROR201301825025- 26 sets (Revenue Offi cial Receipt) ROR201301825050 (unused) BIR Form No.2524 ROR20l30 r 826001- I pad (Revenue Official Receipt) ROR201301826050 (unused) The abovementioned forms were lost during Marawi siege in Marawi City, as reported by Ms. Raihana Menelifa T. Lucman, previously designated as Revenue Collection officer, Revenue District office No. 102, Revenue Region No. 16, Cagayan de Oro City and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID.
