REPUBLIC OF THE PHILIPPINES DEPARTMENT OF FINANCE BUREAU OF INTERNAL REVENUE Quezon C ity March 17, 2008 REVENUE MEMORANDUM CIRCULAR No. 40-2008 SUBJECT : Missing one (1) set of an unused BIR Form No. 0423 � Apprehension Slip TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the missing one (1) set of an unused BIR Form No. 0423 � Apprehension Slip (AS) with the hereunder stated serial number, to wit: APS200100001705 The abovementioned form was reported as missing by Ms. Nonita F. Opao, Administrative Officer III, Revenue District Office No. 26, Revenue Region No. 05, Valenzuela City. Consequently, the entire AS set bearing the said serial number is hereby cancelled. All offic ial transactions involving the use of said form is therefore considered as INVALID. Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid form is found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
