Notice is hereby given of the loss of the original copy from one (1) set of a used but unissued BIR Form No. 2524 -- Revenue Official Receipt (ROR) with the hereunder stated serial number, to wit:

cancelled. All official transactions involving the use thereof is therefore considered as Pangasinan. Consequently, the entire ROR set bearing the said serial number is hereby Revenue Officer I, Revenue District Office No. 3, Revenue Region No. 1, Calasiao, INVALID. The abovementioned copy was reported as lost by Ms. Flordeliza E. Viaje.

: Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid form is found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
