REPUBLIC OF THE PHILIPPINES DEPARTMENT OF FINANCE BUREAU OF INTERNAL REVENUE Quezon City September 17, 2009 REVENUE MEMORANDUM CIRCULAR No. 59-2009 SUBJECT : Loss of four (4) original copies of Tax Credit Certificates issued to BUTUAN HJR INTERNATIONAL CORPORATION TO : All Regional Directors, Revenue District Officers, Employees and others Concerned Notice is hereby given of the loss of four (4) original copies of Tax Credit Certificates (TCCs) to wit: SERIAL NUMBER DATE OF ISSUE AMOUNT R-017432 January 30, 2008 Php 54,293.25 R-017433 January 30, 2008 Php 68,768.19 R-017434 January 30, 2008 Php 93,993.34 R-017435 January 30, 2008 Php 43,636.96 The above-mentioned TCCs were reported as lost by Ms. CARMEN M. CAMASURA of HJR GROUP OF COMPANIES. Consequently, the entire TCC set bearing the said serial numbers are hereby cancelled. All official transactions involving the use of said form is therefore considered as INVALID effective June 21, 2008 when Mr. Teofanes Paradero, company's representative boarded the ill- fated ship, Princess of the Stars, carrying with him the abovementioned TCCs. Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid TCCs are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
