REPUBLIC OF THE PHILIPPINES DEPARTMENT OF FINANCE BUREAU OF INTERNAL REVENUE Quezon City July 29, 2008 REVEN UE M EM ORANDUM CIRCULAR No. 53-2008 SUBJECT : Lost one (1) pad of an unused Accountable Form No. 51 (AF51) All Internal Revenue Officials, Employees and Others Concerned TO : Notice is hereby given of the loss of one (1) pad of an unused Accountable Form No. 51 (AF51) with the following serial numbers: ACF200700245501-ACF200700245550 The abovementioned accountable forms were reported as lost by Ms. Ranulfa Noel Victores, Revenue O fficer III, Revenue District Office No. 33, Revenue Region No. 06, Manila, and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. Internal Revenue O fficials, employees and others concerned are requested to promptly notify this O ffice in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the sa me.
