All information and documents submitted relative with this application are valid, true, and correct; 2. We execute this declaration with the understanding that any misrepresentation, false statement, or failure to disclose requested information may constitute sufficient grounds for disapproval or denial of this application, or revocation of the corresponding Gaming License should such misrepresentation or omission be discovered after approval. 3. We agree to comply with and be bound by the provisions of the latest Regulatory Framework. We further understand that the incomplete submission of requirements shall result in the return of, or non-acceptance of, this application. 4. We acknowledge that the submission of this application form and the documentary requirements do not guarantee approval of the application for conversion to a Unified Gaming License (UGL). The grant of any Gaming License remains subject to the EGLD evaluation and the approval of the PAGCOR Board of Directors. 5. We understand that, upon approval of the conversion, the affected Standard Gaming Licenses (SGLs) shall be cancelled and surrendered to EGLD before the release of the Unified Gaming License and that the required Performance Cash Deposit (PCD) shall be posted with PAGCOR within fifteen (15) days from receipt of the Notice of Approval. 6. We further acknowledge that operations for the mandatory game offerings must commence within one (1) year from the approval of the UGL conversion; otherwise, the license shall be reverted to the corresponding SGLs based on the approved game offerings of the existing gaming venue. 7. We likewise undertake to comply with all conditions, directives, and regulatory requirements that may be imposed by PAGCOR in connection with the issuance, maintenance, and operation of the Unified Gaming License.
