SLTBJECT : Lost One ( 1 ) Pad of Unused BIR Form No. 5 05 3 Taxpayer Information Sheet TO All Internai Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of one (1) pad of unused BIR Form No. 0535 - Taxpayer Information Sheet, bearing the foilowing serial numbers: TIS2 0 t 400298S0 i -rlS 2M 40A2988s0 The abovementioned forms were reported as lost by Ms. Leonarda S. Vicente, Chief, Administrative Section, Revenue District Office No. 36, Revenue Region No. 06, Manila, and have aonsequently been cancelled. AIl official transactions involving the use of said forrns are therefore considered as INVALID. Internal Revenue officials, ernplo)'ees and others concerned are requested to promptly notify this Office in the event that the aforesaid forrns are found and to take the necessary measures to prevent the improper or frauduttent disposition or use of the same.
