REVENUE MEMORANDUM CIRCULAR NO. 50-2005 issued on September 20, 2005 notifies the loss of original copy of an unissued BIR Form No. 0421-Tax Verification Notice (TVN) with Serial No. TVN200200127402. Said form, which was reported lost by Ms. Jocelyn B. Briones and Mr. Guillermo M. De Castro both of RR 8, Makati City, has been cancelled and all official transactions involving the use thereof is considered invalid.
