REPUBLIC OF THE PHILIPPINES DEPARTMENT OF FINANCE BUREAU OF INTERNAL REVENUE Quezon City May 14, 2013 REVENUE MEMORANDUM CIRCULAR No. 42-2013 SUBJECT : Lost Four (4) sets of previously cancelled BIR Form No. 0402 � Authority to Cancel Assessment TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of four (4) sets of previously cancelled BIR Form No. 0402 � Authority to Cancel Assessment, bearing the following serial numbers: ACA200900000103 ACA200900000104 ACA200900000105 ACA200900000107 The abovementioned forms were reported as lost by Ms. Gean M. Dienzo, of Revenue District Office No. 28, Revenue Region No. 7, Quezon City. All official transactions involving the use of said forms are therefore considered as INVALID. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
