REPUBLIC OF THE PHILIPPINES DEPARTMENT OF FINANCE BUREAU OF INTERNAL REVENUE Quezon City June 8, 2012 REVENUE MEMORANDUM CIRCULAR No. 27-2012 _ SUBJECT : Missing One (1) unused set of BIR form No. 0425 � Notice of Disqualifi- cation under NO AUDIT PROGRAM TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the missing one (1) unused set of BIR Form No. 0425 � Notice of Disqualification under NO AUDIT PROGRAM (NOD) with the hereunder stated serial number, to wit: NOD200600004291 The abovementioned form was reported missing by Ms. Lydia N. Brion, Administrative Assistant III, Assessment Division, Revenue Region No. 9, San Pablo City, and have consequently been cancelled. All official transactions involving the use of said form is therefore considered as INVALID. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
